MDS / Care Plan Nurse

University Park Rehabilitation and Healthcare Center is seeking an MDS / Care Plan Nurse to join their team in Fort Wayne, IN. The role involves the accurate and timely completion of Medicare/Medicaid case-mix documents to ensure appropriate reimbursement for care and services.

As an MDS / Care Plan Nurse, you will be responsible for assessing the health status of new admissions, ensuring the completion of MDS assessments, coordinating interdisciplinary participation, maintaining schedules for assessments, and assisting in formulating care plans. You will also evaluate care plans, manage level of care changes, and ensure compliance with state and federal regulations.
